Please have your health care provider complete the following forms:

• Record of Medical Exam, Part II: School Health Assessment
• Maryland DHMH Immunization Record
• Maryland DHMH Blood Lead Testing Certificate (for students entering PreK3 through 1st grade only)

Please submit copies of the following documents:

• Your child’s birth certificate
• Your government-issued photo ID
• Two (2) proofs of Baltimore City residency:
  • Signed lease agreement
  • Mortgage statement
  • Gas, electric, water, landline telephone, or cable bill
  • Paystub
  • Bank statement
  • Social Security/Department of Social Services letter
• If applicable, your child’s IEP, IFSP, and/or 504
• If applicable, custody or guardianship paperwork
• **For PreK3 and PreK4 students only** – income information for one month (pay stubs, W2, 1099)

NOTE: For children already enrolled in Baltimore City Schools, please request a withdrawal packet from your current school’s office. (These may not be available until the end of the school year.)
